Objective Incident Tracking

Whenever production must modify or redesign a sold project, the incident should be recorded using objective facts rather than opinions.

For example:

  • What happened?

  • What was the customer impact?

  • What was the production impact?

  • What was the financial impact?

  • Had the designer completed the applicable training?

  • Was the standard clearly documented?

  • Could the CAD software have prevented the mistake?

The purpose is documentation—not blame.

A Transparent Point System

Many successful organizations use objective point systems rather than subjective discipline.

For example, attendance policies often assign different point values to different types of attendance events. Employees understand the rules in advance, the calculations are consistent, and managers are less likely to be accused of favoritism because the process is transparent.
